Requirements: Please list valid email address when enrolling. The list of required items will be emailed prior to orientation with more detailed instructions. Please bring to orientation: government issued photo ID, proof of Social Security number, OSBI name-based background check with sexual and violent offender within the last three months, negative TB skin test within last six months, flu shot (in season), completed medical release form, immunization record, and AHA BLS for Healthcare Providers card.
